Razionale scientifico

Impressive therapeutic progress has been made in the treatment of patients with metastatic breast cancer in recent years. In particular, the advent of cyclin-dependent kinase inhibitors, novel targeted therapies and, more recently, highly effective antibody-drug conjugates resulted in a dramatic increase in tumor responses and in higher survival expectancy across all breast cancer biologies. However, clinical trials investigating novel treatments for metastatic breast cancer typically enroll patients that are selected on the basis of strict clinical and biological criteria, such as better performance status, higher motivation to receive systemic therapies, absence of severe comorbidities and more controlled concomitant pharmacological treatments; in addition, patients enrolled in clinical trials are clinically and radiologically re-evaluated according to tighter schedules, thus allowing a more rigorous and timely assessment of tumor control and of treatment toxicities. By contrast, patients treated in clinical practice are much more heterogeneous in terms of clinical and biological characteristics, as well as in terms of tumor re-evaluation schedules. For these reasons, the translation of novel treatments for metastatic breast cancer in broad and heterogeneous patient populations, as well as the assessment of safety, feasibility and effectiveness of these treatments in clinical practice is a challenging issue, which requires dedicated studies and discussions to understand how these novel treatments actually impact on patient survival, quality of life and global well-being. This course aims to discuss: 1) the available evidence regarding the effectiveness of novel treatments for metastatic breast cancer in clinical practice; 2) the impact of these novel treatments on patient survival, quality of life and global well-being; 3) how patients perceive the advent of more effective systemic therapies in terms of impact on physical, emotional and cognitive well-being, social and familiar interactions, and career.
