Cancer is a dynamic and evolving disease. Its initiation, progression, immune escape, and therapy resistance are driven by complex biological, molecular, and ecological forces, creating a constantly shifting landscape that challenges modern oncology. Understanding and intervening in this complexity requires integrating evolutionary biology, tumor ecosystem analysis, and state-of-the-art technologies together with novel approaches of precision oncology. The scientific program of the 66th Annual Meeting of the Italian Cancer Society is designed to reflect this conceptual shift and bring together clinical and preclinical researcher to discuss the most novel and compelling evidences that might help in designing more active and less toxic treatments. The meeting will be therefore of interest for both preclinical and clinical researchers, working in different areas of cancer research. Over three days the scientific program will see the participation of Italian and foreign world-renowned scientist to integrate clinical and preclinical presentations on different pathologies including but not limited to women (breast and gynecological), gastrointestinal, prostate, lung and pancreatic cancers. This meeting brings together evolutionary biology, technology, and clinical innovation to redefine how we understand and treat cancer. We will examine tumor evolution and ecosystems, revealing how microenvironmental pressures shape heterogeneity, immune escape, metastasis, and resistance. We will showcase transformative technologies — multimodal AI, spatial omics, digital twins, liquid biopsies, and advanced imaging — that are reshaping how cancer is mapped, modeled, and predicted, accelerating translation to the clinic. Finally, we will discuss of precision oncology, focusing on adaptive therapies, rational combinations, and biomarker-driven strategies that exploit tumor vulnerabilities uncovered by evolutionary and multi-omic insights. On the opening day SIC will hold an open session dedicated to survivors, patients, caregivers and all stakeholders involved in the cancer care. In an interactive round table, it will be discussed why and how research can impact on cancer prevention, treatment and patients' quality of life.
